1996 chrysler town & country has bad vibration under slight load (uphill)
RPM stays same, motor mounts have been replaced.

  2. Very commonly caused by a worn cv joint, usually inner joint. Usually
cheaper to buy rebuilt halfshaft(axle) assembly.

It could be a leaking MAP Sensor. I have a 1996 T&C myself, and it can also
cause a feeling of a miss. Also have you changed your tranny filter lately?
The 604A's can be very touchy.

Possibly a broken cord in one of the tires (rotate to eliminate) or a faulty
inner cv joint ( normally on the passenger side)
